[Plugin: WP Customer Reviews] Not creating entry in database

I have WP Customer Reviews installed and working but when a review is posted, nothing shows up to moderate. I checked in phpMyAdmin and there is nothing in the database for WP Customer Reviews. It seems that it wasn’t created. Any ideas? I have tried uninstalling and reinstalling but no luck.
