[Plugin: WordPress SEO by Yoast] Title Rewrite Doesn't Work- Adds Category Description?

I’ve done everything I know how… I’ve read the posts in this forum, tried changing my header.php… everything. My indexing is getting messed up because I can’t figure this out.

When I try to add add the correct code ” <title><?php wp_title(”); ?></title> ” to my header.php file, it shows some of the script at the top of my website. So I opted instead to use the forced title rewrite.

When I do this, my titles are still messed up. Posts show what category they come from instead of the post title and my homepage also has a tag on the front end of the title showing my “Articles” category.
