Need simple Paypal payment (not purchase) solution

I am a complete WP newb.

Now that we have that out of the way, I am migrating a corporate website from Website Tonight to hosted WP. Everything has gone relatively smoothly until I got to the Paypal part. We accept payments for bail bonds online. Great idea except WP doesn’t like payment buttons with dropdowns and blank fields. This is what I require:

Type of Payment – dropdown (Initial Payment, Installment Payment, Other Fee)
Name of Bondsman – dropdown or blank field
Name of Defendant – blank field
Amount of Payment – blank field
Pay Now Button

After mind-numbing hours trying Plugin this and that, I give up.

Can someone please explain to me a simple way to accomplish this?
