Custom Comment Fields

Hi,
I might explain first what I am trying to do. I send an email through mailchimp to find out if their accommodation is available this weekend. They login and just comment at post whether they do or not plus extra information. I want to tailor these comments. So include the hotel name, phone number and a radio button – yes or no if available. Depending on the answer I would love to change the css to highlight red/green the comment box. Any ideas on how to do this? Thanks!
