Attachment in e-mail to customer

I have spent a few hours and cannot figure out how to get the attachment to work for the e-mail that gets sent to the customer. I have check faq and google. I want to upload an attachment and have it in the e-mail that is sent out please help if you can. the site is http://byuidating.com/
