Forms that will do lots of things, and go in blog post?

Hi

Ok I am after a plugin that will allow me to add as many different style inputs, like small text box’s, tick box’s, large text fields and drop downs etc

also allow you to upload pics or/and vids

and be secure like capta and only selected file formats

is there a plugin that does all this? I see some to some but not all?

its got to go in a blog posts?
