[Plugin: Share and Follow] Follow tab does not appear

Hi, I just installed the plugin but it’s not appearing on my blog: http://www.movebeyond.net/know-no-limits/

Having read some of the documentation I think I know why but need some help to fix it. It says the plugin puts a div in the header to make this work, but my blog is customised and integrated within an existing html website structure, so doesn’t use the standard WordPress files for the header and footer etc.

Can you tell me what the div html code is and where to place it in the header so I can include it manually in the file?
