[Plugin: WordPress Download Monitor] simple question about adding a download to post – please help

Hi there, on this page here >> http://www.littlesplodgers.com/pre-school-sessions/ you will see that half way down i have a download a registration form image. Currently i have simply uploaded a pdf to my media library. However, I am wanting to use the download monitor to track the downloads for this and further additions to the site.

Could someone please explain how/where i get the address for the pdf in the download monitor and add it to this image so when it is clicked the pdf downloads like it does currently but the number of downloads is tracked.
