[Plugin: WordPress Forms] Error when trying to display form

I have published a form (I love the back end of this plugin, so nice).

I am getting this error when I try to visit the page:

Catchable fatal error: Object of class WP_Error could not be converted to string in /home1/awkwardi/public_html/wp-content/themes/Minimal/includes/breadcrumb.php on line 20

what’s the deal? how do I fix this? Something to do with breadcrumbs not recognizing the form?
