Rating system on template?

Here’s my scenario:

I have multiple game profiles on my site. Each profile is a page that is generated from a template file. Basically, I use a private page that is generated from its own template….this private page is a data entry form. Information I put into the form gets sent to its own game profile via the template file used to generate game profile pages. What I am wondering is how can I put in a code so that a rating system will be shown on each game profile generated by the game profile template? Each game profile page is generated from the template so any edit I make to the template will show up on all of the game profile pages. This is what confuses me….I want to put a rating system into the template so that each game profile page has its own rating system but I want it to be so that when a user rates a certain game profile, that specific game profile will have its own rating and the rating wont affect the other game profiles.

Any ideas?
