[Plugin: Receive Links Plugin] I get errors

Following the very simple instructions, I added the plugin. Upon activation I get the following error.

The plugin generated 1512 characters of unexpected output during activation. If you notice “headers already sent” messages, problems with syndication feeds or other issues, try deactivating or removing this plugin.

It still shows up on the widgets. I tried adding the widget to the sidebar and I get the following errors on the site:
