When looking at my google real time analytics page I can see several 404 errors. It is not every time but it happens quite often. I cannot imagine what in the plugin code could trigger something like this. May be you could try entering no ad code in the text boxes in the plugin admin and see if the error occurs. If not, try with some simple text (e.g, “this would be the ad code”) in the text boxes and see they come through ok. If these steps work ok, then it would seem that the ad code you have entered is somehow triggering the 404 errors.
