Custom permalinks changed to default

I was using the customizable permalink plugin and out of nowhere the custom links are gone and have reverted to the “ugly” ones. I also tried the Custom Permalink (Mike Tyson) plugin and that doesn’t work either.

Anyone know what could cause this? I had not done any editing or plugin changes beforehand…
