Can't remove link "Edit Form"

The signup form works great.
The only issue I have is that after the submit button, there’s a “Edit Form” link, with a URL:
/wp-admin/admin.php?page=yikes-mailchimp-edit-form&id=1

Any clue why is that, and how can I remove it?
