That says “Don’t allow new users to add themselves this site, add them manually.”
That means they can’t join the site, so why would the form show?
Yes sorry that’s obvious I suppose, I had it on Manual initially. It is set on Manual now and still no registration form. There is also no form when set on Automatic.
When you’re logged in, what do you see?
I wonder if it’s a theme or another plugin clashing.
When I’m logged in and I go to https://mentalhealthsa.org.za/register/ then it says “Howdy Member!”
The following plugins are active:
– Business Directory Plugin (doesn’t clash with your plugin tested on my local network), I need your plugin for the directory.
– EmbedIt
– Facebook Open Graph, Google+ and Twitter Card Tags
– Fast Secure Contact Form
– Meta Slider
– Rich Text Tags, Categories, and Taxonomies
– Spots
– StatPress
– WP-Piwik
Nothing changed when I deactivated these.
Network wide installed
– WordPress MU Domain Mapping
– BWP reCAPT is installed
The theme & your plugin works fine on my local network. I’m at a bit of a loss. Thanks for your time.
Does the server have any caching or such going on?
What happens if you turn WP_DEBUG on?

Im having exactly the same issue that Velofreak. All plugins deactivated.
Theme:Twenty Fifteen
Site Membership: Automatic
Registration per site: Yes
Created a first level page with shortcode.
When I’m logged in and I go to https://dibider.bts.com/auto-registro then it says “Howdy Member!”
But without login in the form is not displayed, only the “Register for an account” button is shown and clicking on it just reloads the page.
Any idea or suggestions?

Which shortcode? There are two. [join-my-multisite]
is to register and that should send you to https://dibider.bts.com/wp-signup.php
I just tested on a clean Multisite and it worked as expected.
Try turning on debug, though I noticed you’re on IIS and https://dibider.bts.com is NOT a WordPress page, so there may be something with your server setup.
