Strange bug

Hello, I would like to get some help in a very strange problem.
I already use the plugin in other projects and for the first time I see this problem.

I installed the plugin usually clicked on “New Form”, the window with the four model options appears normally but when you click on one of the options, simply nothing happens, nothing.

I checked the Developer tools in Console and it shows errors in the image below.
