You need to deactivate all you plugins, then switch to a standard wordpress theme. When done write me again and I’ll make a test.

Anyway I do suggest you to install the plugin on a development website, make your tests and when everything wirks deply on production website.
the problems you are facing are 100% related to the theme in use.

Hello,
It’s not resolved yet.
I’ve followed your guide ..made the test. but the issue is still there.
On removing custom services it disappears and on adding services it appears again.
Your booking custom form is clearly broken by the theme in use.
