This is most likely caused by a JavaScript error in another plugin, which is cascading down and effecting the rest of the JavaScript on your page.
The easiest way to troubleshoot this is to switch to a default theme and deactivate all other plugins. Then you can start reactivating them one-by-one until you find the source of the problem.
This has happened a lot in the last few weeks because WordPress updated it’s version of jQuery. The Restaurant Reservations plugin works fine with it, but many other plugins haven’t been updated properly and are causing knock-on effects.

I’ve just had this problem. Thanks for the advice – I switched templates and it worked right away, so the template was out of date. After a quick check, there was an update and now all working fine.
