Is there not another solution that doesn’t involve throwing out other useful tools to get this useful tool to work?
Without knowing the root cause of the troubles you are having, no. Isolate the cause and go from there.

Howdy rameez,
This is the only plugin installed on wordpress. I have the default theme laoded in and have not done anything except installing your plugin
This is not reproducible under those conditions. When you say “default” theme, what is the name of that theme?

Can you use your browser’s developer tools and see what JavaScript errors are in the console?

Thanks for that. Can you get the error message from the error log? Also, what version of PHP is used on the site?
hi i am using latest php 7. Here is latest the error log message.

I haven’t got a PHP7 test environment (I guess I need to work on that) but that really shouldn’t make a difference. Can you make a screencast of the steps leading up to this – perhaps with something like http://recordit.co and send the link to that screencast so I can see if I can replicate the issue on my test environment.
hi will here are the screencasts you asked for.
I first run xampp and start apache and mysql.

This is the second step

Let me know if you need any thing else from my side.

Hey, thanks for all of that. I set up a php7 test environment and I am now able to replicate this. I’m not sure when of if I’ll be able to fix it but it’s on my “to do” list.
update i changed my php version to 5.xx and now it works. Everything else is the same.
I was having the same issue with the java script loading error. I check php it was 5.5.
Awesome survey has a conflict with another plugin: EasyRecipe
Version 3.5.3208 | By EasyRecipe
I hope this helps someone. Jim R

@ramxis02 So, unfortunately I think this means the end of this plugin. I can’t make it compatible across all the versions of PHP. I will be removing the files from this repository soon. It will remain on github but will not be maintained.
