The most likely cause is a theme conflict. Can you try switching to a default theme, like TwentyFifteen, and see if that fixes the issue?
If you want to add a link to the booking form on your site I’ll take a look.
Thanks for your response, I tried it with a theme Default is working properly, but when I use it with the theme DIVI, the stylesheet file is not loaded. Then I also noticed that if the plugin is active, is used on the page, the images are not uploaded.
All this is strange because your plugin has never given problems of compatibility with the theme DIVI.

I’m afraid I don’t know the DIVI theme that well. Have you tried contacting the developers of the theme to ask them what’s going on? It’s most likely a particular setting or configuration that’s causing the issue, and they’ll be better positioned to determine the source of the problem.
I have not contacted tech support, because I used your plugin sometimes with Divi, but perhaps the latest theme update causes this problem. I try to ask the support of Elegant Theme and then you upgrade.
If I find the problem and the solution I will share with you, so if other users have the same problem, you can immediately give a solution.
