Need Form plugin with upload feature

    Hi,

    I am setting up a WordPress site and need a plug-in which allows me to make a form by drag and drop and has the capibility to allow users to upload files to my server. Then the details are e-mailed to me on submission of the form along with link to uploaded files…

    WordPress is huge and so there might be a plugin like this. So I just thought that the Forum community could help me out. Thanks.

    Eg:
    User fills- Name, E-mail, Phone No.
    Attaches- main.zip file
    Presses submit- The zip file should be uploaded to my server and I should get a mail with the data (Name/E-mail/phone) and the link to zip file.
