This plugin is broken and now I can't delete the uploader

I installed this plugin thinking it would help with iOS uploads. It didn’t work on my iphone, the “Add Files” button did nothing. It also didn’t work with an explicitly listed file format (my list is this: jpg, gif, png, pdf, avi, mkv, mov, mp4, m4v, DivX, flv, ogg, ogv, mpeg-4, mpeg, vob, wmv and it wouldn’t accept an mp4 file saying it was an invalid extension) when I tried to upload them from my browser. Then when I tried to delete the uploader from my Gravity Forms form the delete button wouldn’t work and I couldn’t get rid of it. I tried disabling the plugin and now the uploader doesn’t appear but the old uploader title is still there.

I am so frustrated, because of this plugin I have to scrap that form and rebuild it, a couple hours worth of work.

It’s total crap. Really haven’t had such a bad experience with a well reviewed plugin in years.
