    If you would like to show a description of your form, you have to add description=”1″ to your shortcode.
    If you use button Insert a form, just check select box “Show Description”

    So shortcode will looks something like [formidable id="25" description="1"]

    It is useful for example if you would like to show some text like “You can contact us using the form bellow” if you use ajax and after submission your form show confirmation message.
    If you add that text normally in WP editor to the page above your form shortcode (above your form), it stays there with a confirmation message, what makes not sense after submission.
    If you use some text as a description of the form, then this text disappear when confirmation message showed up.
