Hi – thinking of buying the FF plugin.

1. Will it work as a questionnaire?

2. Can each completed questionnaire be emailed to me?

3. Can each completed questionnaire be emailed to me as an emailed PDF attachment?

4. When the form is emailed to me, does it include the questions and answers of that user?

5. Do you have some URLs of any long questionnaire built with FF?

1 & 2. Yes.

3. This is not a built-in option, but there are a couple of third-party options. There is a PDF add-on:
http://www.formidablepro2pdf.com/
that will populate fields in a PDF and that can be attached to the email. Also, I believe this may do what you’re looking for:

4. By default, it does. You can customize the email message though.

5. We don’t have any demos of a long questionnaire right now.
