I’m sorry about the delay. As you probably know from my support forum post, I’ve been away on a family emergency.
First, are you talking about the plugin at https://wordpress.org/plugins/form-maker/?
Second, what exactly do you mean by crashed? Are you getting programming error messages, a weird screen, or something else?
And, finally, if I use a fresh WordPress install, add Seamless Donations 4.0.8 and a new download of Form Maker, is there anything I need to do to set up Form Maker (or Seamless Donations) to cause the problem?
If I can repeat it, I’ll see if I can fix it.

Yes that’s the form maker plugin i’m using.
By crashed, I mean my form doesn’t load when i have the SD activated. It shows a blank page and some of my page elements don’t load properly too such as drop down menu. But this happens only on the page where i have the form maker loaded. I have made a long form application, and have done some customization into it too, so i don’t want to remove it and install it again.
I deleted the SD and installed the new version as well but no luck. As i mentioned earlier, I don’t want to delete my form maker plugin and re install.
The thing is that the previous version of SD was working fine with the form maker, both were loading simultaneously.
Well, here’s a quick idea. In the Form Options tab, at the bottom, there’s a tweak to turn on form labels. Try that and see if it helps. Let me know. If not, I’ll download Form Maker and see if I run into a snag. My guess is there might be some CSS conflict.

I have already done that while i was working on the css styles of the SD. But then realised that the form doesn’t work so uninstalled the SD.
Yes, please try it with the form maker and see if you get the same problem as mine.

Just to follow up, did you get chance to test Form Maker and SD together?

Yep, it took a little while because I had to spin up a test Web site to try this out.
Here’s what I did:
- Created a new WordPress site (I like using fresh sites so we know what we’re dealing with is the problem we’re testing for)
- Set the theme to Twenty Thirteen, so we know what we’re dealing with theme-wise
- Installed Seamless Donations and used the default Form Option settings
- Created a Sample Page containing just the [seamless-donations] shortcode
- Installed Form Maker from the WordPress repo
- Left Form Maker with the default theme, and went to the Manager tab, and copied the shortcode for the first form, [Form id=”1″]
- Created another page and on this page, pasted in [Form id=”1″]
- Visited the Seamless Donations page and it displayed properly
- Visited the Form Maker page and it displayed properly
So, as far as I can tell, they work together. My guess is it’s something about the theme, or possibly something about a form configuration that I’m not repeating.

So, do you have the version 4.0.3 of the SD to send me please? Coz that was working for me. Unfortunately I don’t know how to find it on my laptop, maybe it’s removed or replaced or something. I will work with the previous version of it.

I would definitely recommend you look into your theme or other possible CSS conflict because a boatload of bugs have been fixed since 4.0.3, including some PayPal-related bugs. Then again, if it works reliably for you, hey, go for it.
