I have the opposite problem! I have it set so that anyone can reply, no moderation, yet they are all moderated, even my own!!
I came here to see if anyone else had the same problem. Heh, sorta not exactly but close enough?

Well, here I go again posting about the fix to my own problem.
I was only looking into my wp-admin files when the problem turned out to be in my main blog directory.
The problem was in the file:
wp-comments-post.php
I just reinstalled this file and now it’s working.
I’ll try to look into some more what changes I had made to this file that caused all comments to be automatically approved. Or you can email me at anna at mustardzone dot com if you want me to send you the file that allowed all comments to go through without approval.
Well, I haven’t had any problems with comment moderation and approval, other than the fact that it sent me 46 different emails as the result of a spam attack . . . Does anyone know of a plugin or hack that will consolidate this into one email at specific intervals during the day? (Or is it time to break out the PHP coding hat?)
