What plugin can i use to insert a contact form into my post?

I’m making a “Tip Us” page where users can anonymously tip messages or files to us. It’s basically a contact form im looking for that gives the option to attach a file to submit. What plugins or methods can i use to insert one of those contact forms into the post?
