Plugin or other way to create zipcode search on site

I’m thinking about adding a plugin or something that can create a searchable database on my site, http://www.helpteachingreading.com.

Basically what I would like is for a user to be able to input their zipcode. In return, I’d like them to get a chart that lists tutoring companies or individuals who tutor in their zipcode.

I don’t want a map showing where they are located. Just a chart that lists the relevant tutoring businesses in that zipcode. Anyone have a plugin or know how I could make this happen?
