I have suppressed the top WordPress Toolbar, using another plugin.
How can I introduce a LOG OUT button, other than the one I’ve suppressed?

There are a few ways to do that.

If it is in a menu or some other link, just make the link http://yourdomain.com/?a=logout

There is also a shortcode you can use: [wpmem_logout] (described here: http://rocketgeek.com/plugins/wp-members/users-guide/shortcodes/other-shortcodes/ ).
