I’m not sure what your situation is and I’m Gonna need some more info: What is the end result you are looking for? How many files are you dealing with?
Sounds like you are trying to upload file outside of the media library. This isn’t necessarily straight forward. Are you running any image plugins?

Hi AJ and thanks for replying. Hopefully I can clarify things a bit.
I originally uploaded around 70 images to the library using the uploader and all seemed well however a change was requested which affected each of those images so I edited each of them in Photoshop and saved them on my hard drive with the same filename thinking it was simply a case of uploading them as before and that they would overwrite / replace the originals. I now see that that is not how WP does it and that the best I could hope for would be that the new file would be added with an altered filename. Fine. However this didnt happen either. What happened was that WP either refused to accept the new files with same filename (if I change the filename before upload it does) or does accept them (they seem to upload alright and as normal) but then automatically replaces new with old version even if I have physically deleted the old unwanted version before uploading the new one.
I am sure it is something simple but it is just one of those little things that gets to be an annoying problem that I just cant quite understand and would like to.
Here is a list of plugins I have installed. I have tried deactivating them but it doesnt seem to make a difference.
