Integration with a Toolbar for Signup Form ("Hello Bar" Style)

I’ve hacked together some styles and code to turn this plugin into a Hello Bar like signup bar. Thanks for making such an excellent plugin!
The steps involved are basically:
- Setup MailChimp for WP
- Integrate the form shortcode with your theme
- Wrap an appropriate set of styles around the form
- Have a free Hello Bar!
