Form fields not visible / Unable to to fill form

That issue exist because of the CSS conflict with your actual theme.
For fixing of that issue you need to make this small customization.

Please open this file http://judellaconquet.com/wp-content/themes/dynamo/style.css

and add to the bottom of that file this code:
