    It looks like you have not “published” your form which is why you are seeing the request to login to Google. The form needs to be published or shared with other users. I typically choose “Share with others who have link” or something like that as it allows the plugin to pull in the HTML of the form without having to login to Google.

    When you click on the link I gave above to the form, you should be able to see the form fine.. It works fine when I embed it directly into the wordpress page.

    Just not using the plugin.

    Any ideas?

    The permissions on your form aren’t correct – I cannot see it unless I login to Google. That is the problem. If you can’t bring up the form using the URL when you are not logged into your Google Account, the plugin will not work.
