    Hello: I had a programmer develop this page for me:

    I can find the page in WordPress, but I can’t find the code for the survey or even see where that survey is located within my WordPress. Can you direct me where to look.

    The survey within the page asks for the phone number but doesn’t send it to me so I need to edit the code. Also, it asks for the email address but puts my address in rather than the person’s address who is completing the survey.

    check which plugins you have installed, the survey may be created in one of them and then added to the page as a shortcode. you’ve got contact form 7 on there so it might be that…

    Thanks Jack: I looked in there and the mystery is that every other form is in Contact Form 7 plug-in except the form I’m looking for. I’m not a programmer, but would the source code of the page give us any clues if I looked at it?
