Audio and Video formats no longer working

Hello I’m having serious issues with my post formats, I use the audio and video fromat alot but just noticed today that they are no longer working, when I click on the audio/video format the part I’m supposed to put my embed code/mp3 url no longer displays.

The worse part is that this has now affected all the videos and audios I’v uploaded on my site before now as they are no longer displaying under their posts. I recently installed the ‘WordPress SEO by yoast’, don’t know if this might be resposible cos I have deactivated and deleted the plug in but the issue still persists.

Please help resolve this issue as soon as possible.
