extra fields in form

Hello,
Thanx for the plugin,
all works well
although the widget seams to be picking up a couple of extra fields after the vscf_sum.
vscf_firstname
and
vscf_lastname
but they don’t have labels.

Just wondering where they come from and how to remove them.
