Custom HTML form – doesnt pass checkbox data

    Hi,

    On my custom form I’ve got a several checkboxes showing services:

    <div><input class="ccf-tooltip-field" title="If you would like to book an additional service please select here." name="additional_services[3]" type="checkbox" value="Gold Service" /> <label class="select" for="additional_services">Gold Service</label></div>

    however even if this is checked the email received doesn’t show the data…

    Any ideas?
