Suggestion regarding to the plugin

Dear Author or Support of “Quick Whois”

I shared my problem at another page ( wordpress.org/support/topic/its-broken-17?replies=2 )
then i discovered that ticket was resolved. However,
The plugin is not working in my website. I followed each steps carefully.
1. I changed my theme, activated another themes but no luck
2. I deactivated each plugin except this one and checked the result but i didn’t get result.
3. I uninstalled->deleted-> re-installed the plugin but that is not working.

Please check this and try to give me suggestion.
