CTA Template Jquery Not working

    Hi,
    Recently I’ve installed CTA plugin and try to customize “Clean CTA” Template. I would like to create template with slide up/down on mouseover.
    but I’m not sure where to add jquery code.

    I added it on template index.php but it is not working.
    Please help me
