A 404 error is fired with a view to not make the custom 404 error page accessible directly by its URL. Usually the error page is not used elsewhere. This is intended and not an error. Take a look at https://wordpress.org/plugins/404page/changelog/.
Version 2.1 is just about ready and will be available for download soon. The final version includes an option to deactivate firing an 404 error in case of directly accessing the page to achieve exactly what you need.

Thanks for your work.
Tested on a first website with WordPress 4.5 and Divi, with 404 Error firing deactivated and standard mode, everything seems all right.
I will continue to verify it’s all good with my other websites.

I confirm it seems to work everywhere.
Except when Polylang is set up, then the compatibility is a must and the error page does not keep its full width format, it reverts to the blog format.
In this case, I have to stay in the 1.4 version…

I have the same issue. Using Divi theme and Polylang and the custom 404 page is appearing as a blog post.
