I have the exact same issue, on the related site MultilingualPress creates always two posts, where you can delete the first of those. The second one seems to be the linked post.

Any hints from the MultilingualPress guys ?
Are these sites all really set as “related” in the network settings page and do they have an assigned language?
In my case, they are set as related and have an assigned language (English and German).

Yes, in my case too, they have a language and a relation, set in the MultilanguagPress tab in the sites admin panel here : host.domain.com/wp-admin/network/site-settings.php?id=2&extra=mlp-site-settings
They don’t have an alternate title though, nor flag url.

can you provide a step-by-step guide to reproduce this?
Please be as specific as possible.
Are you creating the original post and the translation in one go, or do you add a translation to an existing post?
Do you save the post as draft or do you publish it?
Is this the case from all sites, or only from a specific one?
Is this a Custom Post Type, or just Posts?
Are there any other plugins (also MU plugins!) active?
Does this still happen with all other plugins turned off?
Plus anything else that might help.

I was able to reproduce this on my own.
I think I solved this in the 2.4
