Need Theme Name

Hi,

I want to setup a wordpress website, where there will be two parties.
Customers and agents. Once the customer come on my website, he’ll select respective dropdowns and fill his information. According to his details, the qualified agents name will show up on last page along with that the agents will get the email/sms with customer information. Is there any theme or plugin available?
