    Hi Henrik,

I have two sites with Google Authenticator plugin.
On one of these websites, today I noticed, the authenticator box on the login page didn’t appear anymore.
To test what this was caused by I updated plugins on my other website. (I updated iThemes Security and WordPress SEO… my guess is that iThemes security caused the problem).
After updating these plugins the authenticator box also is gone on the login page of the second website.
I updated iThemes Security from version 4.0.5 to 4.0.14, with 4.0.5 there was no problem.
    Your help would be appreciated.

I am also having this issue. iThemes 4.0.7 broke my Google Authenticator to where I couldn’t log in to my site without deactivating the GA plugin. I recently updated to 4.0.16 and with GA activated I can now log in, but there is no option to enter the two-step code.

Waiting for a solution from either Google Authenticator or iThemes Security.
At least both plugin-makers should know of
    this issue.

Which settings have you enabled in the iThemes Security plugin ?

    Several setting.
As you can read in this topic http://wordpress.org/support/topic/enable-hide-backend-google-authenticator probably the “Hide backend” option probably causes the problem.
But I’m not sure of that.
Everything worked well (with the same settings) with version 4.0.5 of iThemes Security.

So someone should try not enabling the Hide backend feature I guess.

    How does that help?
We (at least I) want to hide the backend and use Google Authenticator as well.

I don’t think that’s possible, apparently iThemes complete takes over the login page.

Hmm I got it working with hidden login on Nginx now.

I can agree that it may be an iThemes Security problem, but it worked together with Google Authenticator in all previous versions (until 4.0.5). So also when hiding the backend.

    Best regards.
