You need to speak to the plugin maker of Simple 301 Redirects because everytime you release a new version of Yoast SEO, it wipes out all the 301´s set using simple 301 redirects.

I suggest you have a look at the Redirection plugin which I have found to be quite stable and doesn’t conflict with WPSEO.

Simple 301 Redirects are doing their redirects in a non-typical way which is probably the reason things go wrong.
