The issue was because your username field in ask question page was missing. Can you provide us the link of your ask question page for further checking?
Hi, I have the same issue (missing username field) in custom template. On other templates it is working OK. I can`t provide a link because site is on Denver now. May be you can tell me the solution of this problem without looking at website?

I too have the same problem when I add submit form by shortcode. There should be a missing function on shortcodes.php file but I really don’t understand.
Hi guys, Thank you for your feedback, we will check the shortcode, fix and release an updated version as soon as possible this week. At the moment, you can set-up a page as the submit question page and use it instead of shortcode.
