I had this same problem and it seems to be a problem when using AJAX on the form. Try turning off AJAX and see if that solves the problem.

I have yet to look into using this with ajax submission so i’m afraid i can’t comment on that yet.
There are a number of things i want to add to the plugin so i will add this to the list for future updates.
For the meantime i would recommend disabling ajax form submission until i have chance to look into this.

Thanks for this plugin… really looking forward to the vimeo pro / plus support if you are making it. this (disabling the ajax option) seems to have fixed the issue I was having with the form not submitting.

Are you getting any other javascript errors? I would bet that another section of javascript is erroring out and causing the pluploader plugin to fail.
Take a look with firebug console or something similar and let me know.
