The word “Fieldset” is just the name of that field. You can remove the name there by clearing it out from the VFB admin.
As far as the “brackets”, it looks like you have the shortcode wrapped in a <code> tag. Go into the Code view and make sure the VFB shortcode doesn’t have any extra tags around it.

I’ve worked with webhosting & internet provider to set up my email through outlook & it’s perfectly configured & working. I also corrected the email address in visual form builder but I still can’t receive any emails. I’m trying to avoid any additional plugins since currently everything else is working well. Any suggestions?
