Plugin breaking site when activated

I have been using this plugin happily for well over a year or more, but recently when activated it breaks my site. I found this out through a process of elimination in my database to see why my site suddenly had a white screen. I have made no changes to the site and have no idea why it would suddenly have broken. I do not update automatically, so there is no issue there. I was running wp 3.3 when it broke and just updated to wp 3.8, but it is still not working. oldstockdale.neighborhoodcrimealert.net
