Plugin doesn't work…with Genesis?!

So I followed the directions to the “t” for the “Sample Theme” only to see the the following:

A charcoal gray box with white letters “EMAIL NEWSLETTER” over gray letters “Sign up to receive email updates and to hear what’s going onwith our company!”

And no place to “sign up.” No input fields or anything.

Not sure what I could have missed here as the instructions seemed pretty explicit. What did I overlook? Thanks.
