Close Opt-in Box and Place Cookie Upon Submit – MailChimp

I have a MailChimp opt-in form set up on my page here http://www.humandesignlearningcenter.com/

Design and layout is displaying exactly how I’d like, but because I’m using a parallax site it can be quite annoying to constantly have it there. I’m trying to find a way to allow the user to close the box (so maybe a pop-up plugin but I want it display in the current location, not center of page) if they wish.

Also, once the form is submitted, I’d like to place a cookie so that the form no longer appears and does not appear the next time the user visits the site.

Is there a plugin I can use that will accomplish both of these items? Is there code I can manually add?
